The purpose of this web site is to give members of the California University of Pennsylvania access to information about the CUP Institutional Review Board.
ENHANCED IRB REVIEW REQUEST FORM NOW AVAILABLE FOR IMMEDIATE DOWNLOAD
(02-12-09) The new enhanced form contains the previous Review Request Form, used since 2005, unchanged. However, the new form has been expanded to include a set of three self-assessment checklists to be completed by the researcher. These checklists are designed to benefit researchers by helping to ensure that their proposals meet OHRP and IRB guidelines before they are submitted. It is the IRB's intent that these new forms will dramatically speed the approval process by significantly reducing the likelihood that the proposal will be returned for editing/correction/clarification.
IMPORTANT NOTE: The checklists reflect ongoing OHRP and IRB requirements for approval of proposals and do not represent a change in standards. Indeed they derive from documentation long available on this website.
Use of the new form and checklists will become mandatory for all proposals received after 02-27-09. Until that date, the old forms will be accepted. However because of the significant benefit to researchers, voluntary submission of the checklists is strongly encouraged by the IRB. To facilitate the transition, the checklists have been made available for download separately.
The enhanced IRB Review Request Form, with checklists can be downloaded using the links on the Right side of this page. Both Adobe Acrobat (.pdf) and Microsoft Word (.doc) formats are available. Documents containing the checklists alone (also in .pdf and .doc formats) are also available for a limited time.
Submission of Review Request Forms:
Email submission of Review Request Forms is strongly encouraged by the IRB as it speeds the review process. Send forms (either .pdf or .doc formats) as attachments to: instreviewboard@cup.edu
Hardcopy submissions should continue to be sent only to Campus Box #109.
