The purpose of this web site is to give members of the California University of Pennsylvania access to information about the CUP Institutional Review Board.

NEW REQUIREMENT FOR CONSENT FORMS TO HAVE EFFECTIVE/EXPIRATION DATES (2007)!

Consent forms for all studies (and cover letters for interviews/questionnaires) must contain a statement indicating that the study approval is effective from xx/xx/xx (date) and expires on yy/yy/yy (date). These dates must be printed on each consent form and cover letter and are assigned by the IRB when the study is approved. Generally, the IRB assigns an expiration date that is 1 year from the effective date.
